If you ever need to make a claim you can select the appropriate claim form from
the table below, complete it and send it to us.
There are two methods available, you can download the form as a PDF file, print
it, fill it in and fax or post it to us, or you can fill in an online form and
send it by e-mail. To download the PDF file, you will need the Adobe Acrobat
reader. The reader can be obtained for free from Adobe.
The fastest way to make a claim with us is by filling in the online form. Please
note that because the form is online, we are unable to collect a signature or
have receipts attached. If we need either of these for your claim, then we will
contact you once we have received your claim.
